RepercussÃes cardio-respiratÃrias da manobra de recrutamernto alveolar em cÃes submetidos à lesÃo pulmonar aguda unilateral

RESUMO

Cardiopulmonary repercussion from Alveolar Recruitment Maneuver (ARM) was analysed in dogs, which had undergone right acute lung injury (ALI) induced by 0.9% saline solution lavage warmed at 38o C. ALI group (n=6) had undergone a right ALI, while in ARM group (n=6), underwent recruitment maneuver after induction unilateral ALI. Respiratory mechanical parameters: static lung compliance (CST) and respiratory system resistance (Rrs); arterial blood gas measurements: arterial oxygen partial pressure and inspired oxygen fraction (PaO2/FiO2) ratio, carbon dioxide partial pressure (PaCO2), and alveolar-arterial oxygen pressure difference [D(A-a)O2]; and cardiovascular parameters data: heart rate (HR), mean arterial blood pressure (MABP), were assessed before and after acute lung injury for the ALI group and after recruitment maneuver for the ARM group. Cardiovascular parameters were also assessed during ARM. Histologic analysis was accomplished after ALI in the acute lung injury group and after recruitment maneuver in the ARM group. Both groups showed significantly decrease in Cstat and increase in Rrs, decrease in PaO2/FiO2 ratio, with increase in PaCO2 and [P(A-a)O2] after unilateral ALI, which were reverted after ARM. HR reduced after unilateral ALI and remained unaltered during or after ARM. MABP remained unchanged during the whole experiment. On histologic analysis, it was observed wide areas of alvelar collapse, mostly on the gravity dependent lung regions after ALI, which was minimized after ARM. Alveolar and interstitial edema and the number of cell remained constant after ARM. Alveolar hyperinflation was observed after the maneuver on the left lung non gravity dependent regions. In conclusion, ARM applied to unilateral ALI is capable of improving respiratory mechanics and arterial blood gases data without hemodynamic repercussions.
