RESUMO

Esta pesquisa procurou analisar os efeitos e os sentidos que uma proposta instituinte provocou nos modos de agir e pensar de diferentes sujeitos atuantes numa instituição: os serviços de Saúde Bucal na atenção básica do SUS Campinas/SP. Originou-se do desejo de acompanhar um grupo de estudos e seus sujeitos que vinham discutindo uma proposta inovadora a fim de modificar o modo de produção do cuidado em Saúde Bucal presente em seus cotidianos. Tal proposta, denominada Alta Pactuada em Saúde Bucal, gerou algumas análises reveladoras do e no modo de organizar, gerir, intervir e pensar o cuidado em saúde bucal no cotidiano de oito cirurgiões dentistas nesta rede básica de saúde. Ao longo desta pesquisa, em que a pesquisa-ação foi se configurando e se (entre)cruzando com e na pesquisa-intervenção, um percurso teórico-metodológico foi produzido e exercitado no que estou denominando pesquisa-ação-intervenção. Tendo como quadro teórico-metodológico a abordagem da Análise Institucional, utilizei, como ferramenta da ação-intervenção a técnica do Diário em três diferentes tipologias: 1) Diário Institucional-D.I., 2) Diário de Pesquisa-DP e 3) Diário de Momentos - DM. Para o tratamento deste material empírico, parti de dispositivos analisadores que emergiram dos processos de subjetivação dos sujeitos nos encontros do grupo, registrados nos diferentes tipos de diários e na Restituição como possibilidade de análise das implicações dos sujeitos. Estes acontecimentos contribuíram para provocar rupturas no modo de agir e pensar a Saúde Bucal no SUS Campinas, contrapondo-se com os saberes e práticas da clínica odontológica instituída, seja nestes serviços, seja em seus processos de formação profissional, pois cada um trouxe, a partir de suas experiências vividas, marcas de uma história que os formou e os constituiu como pessoa e como profissional, ou seja, suas implicações. As experienciações da Alta Pactuada, realizadas pelos sujeitos participantes deste estudo, permaneceram bastante demarcadas no campo da Saúde Bucal, fazendo poucas interlocuções com a Saúde Coletiva. Isto porque, o modo de produção de subjetivação do cirurgião-dentista ainda é fortemente direcionado para olhar a boca apenas como um conjunto de "dentes e gengivas". Daí a relevância desta açãointervenção proposta nesta tese, no sentido de contribuir para desterritorializar este tipo de modelagem limitante, potencializando formas instituintes/inovadoras na produção do cuidado em Saúde Bucal Abstract; The present study consists of an analysis of meanings and effects that a project had on the actions and thoughts of different subjects at Basic Oral Health Centers belonging to the National Public Health System (SUS) in the city of Campinas, State of São Paulo, Brazil. The study had its origins in the intention to monitor the activities of a study group and its subjects, who had been discussing an innovating proposal to improve services in the area of oral health care. This proposal, known as "Consented Completion of Oral Health Services," brought about interesting analyses of ways of organizing, managing, intervening and taking care in the work of eight dental surgeons at the centers. During the study, action-research took place and was inter-related to intervention-research. Based on the theoretical-methodological approach of institutional analysis, the diary technique was used as the tool of action-intervention in three different typologies: 1) Institutional Diary, 2) Research Diary and 3) Running Diary. To treat this empirical data I used analyzing devices that emerged from the subjetivation processes of the subjects during group meetings, as recorded in the different diaries, with restitution was seen as a possibility for analyzing the implications of the subjects. These facts contributed to the appearance of changes in the ways of acting and thinking at Basic Oral Health Centers in Campinas, in contrast to the usual instituted knowledge and practice at dental clinics, both in terms of the centers themselves and the processes of professional formation (since all the participants brought with them their own personal experiences, as chapters in the history that took shape and that constituted them as persons and as professionals, in other words, their implications). The action-experience of Consented Completion by the subjects who participated in this study stands out in the field of oral health services, with little relation to collective health in general. This is because the mode of production of subjectivation of dental surgeons still usually consists of considering the mouth as little more than a set of teeth and gums. Hence the relevance of this action-intervention, as is proposed in this article, intended as a contribution toward the de-territorialization of this type of modeling, thus strengthening innovative forms of action in the area of oral health
