RESUMO

The aim of the present study was to evaluate the clinical and microbiological effects of scaling and root planing (SRP) alone or in combination with metronidazole (MTZ) and amoxicillin (AMX) in the treatment of subjects with generalized aggressive periodontitis (GAgP). A double-blind, placebo-controlled, randomized clinical trial was conducted in 30 subjects with GAgP. Subjects received SRP alone (n=15) or combined with antibiotics (n=15; 400mg MTZ and 500mg AMX three times a day for 14 days). All subjects received oral hygiene instruction including the use of 0.12% chlorhexidine mouthrinse solution twice a day for 2 months. Clinical and microbiological examinations were performed at baseline and 3 months post-therapy. Subgingival samples were individually analyzed by checkerboard DNA-DNA hybridization for 40 bacterial species. In both groups, all clinical parameters improved at 3 months. Subjects receiving SRP+MTZ+AMX showed the greatest improvements in mean full-mouth probing depth and clinical attachment level. The antibiotic therapies associated with SRP led to additional clinical benefits over SRP alone in intermediate and deep sites. The SRP+MTZ+AMX therapy led to the most beneficial changes in the subgingival microbial profile. Subjects in this group showed significant reductions in mean counts and proportion of periodontal pathogens such as Tannerella forsythia, Porphyromonas gingivalis and Treponema denticola, and the greatest increase in proportions of hostcompatible species. In addition, The SRP+MTZ+AMX therapy reduced significantly Aggregatibacter actinomycetemcomitans in deep sites. Significant advantages are observed in clinical parameters and in the composition of the subgingival microbiota when MTZ+AMX was associated with SRP in the treatment of subjects with generalized aggressive periodontitis when compared with SRP alone.
