RESUMO

Objetive: To investigate if breastfeeding has a protective effect against overweight in preeschool age. Methods: We reviewed the literature about the hypothesis that breastfeeding has a protective effect against overweight, itâs biological plausibility and the potential mechanisms involved. We also carried out a cross â sectional and a case â control study with children 2 to 6 years old to test this hypothesis and to identify other factors that interfere in this association. Overweight was defined as body mass index (BMI) for age and sex equal to or above the 85th percentile. The main exposition was exclusive or predominant breastfeeding for less than four months. Results: Early interruption of breastfeeding was a significant risk factor for overweight at the cross â sectional study and at the univariate analysis of the case â control study (OR= 1,69; CI 95%: 1,10-2,60; p= 0,02),but not at the multivariate analysis (OR= 1,42; CI 95%: 0,86-2,34; p= 0,17). Maternal overweight, birth weight ≥ 3.500g and sedentarism were the main risk factors for overweight at the multivariate analysis. Conclusions: Our results suggest that if a protective effect of breastfeeding against overweight in preschool age does exist, it is weaker than genetic and other environmental factors. Preventive measures against overweight should emphasize increasing physical activity among children and adolescents
