RESUMO

The purpose of the present study was to measure the plasma concentrations of brain natriuretic peptide(PNB) in patients with seropositive for Chaga`s disease(CD) . CD affects 16 million people in Latin America, of whom 6 to 8 million are brazilians people. PNB is increased in congestive disease, suggesting that the potent vasodilatory and natriuretic properties of PNB are a response to expanded fluid volume in congestive heart failure.It has been established that this increase is proportional to the degree of heart failure, hence its use to quantify the degree of left ventricular dysfunction as a prognostic marker and also to assess the effectiness of therapy.Plasma concentrations of PNB were measured using a fluorescence immunoassay kit (Triage, Biosite). The PNB levels of 25 patients attending the CD clinic of Oswaldo Cruz University Hospital were valuated.The patients were divided in to 2 groups: the first group (G1) comprised 13 individuals seropositive for CD, who were asymptomatics and without cardiac diseases detected by eletrocardiographic, chest radiographi and echocardiogram, The second group (G2) consisted of 12 CD patients with cardiac involvement evidenced by clinical findings and changes in the eletrocardiogram, chest radiographi and echocardiogram. These patients had no other cardiac disease.Levels of PNB in CD patients with cardiac involvement were significantly greater (G1=4,4 and G2=293,09 p<0,01). In neither groups was there any correlation between the PNB levels and sex or age. The levels were directly proportional to the functional NYHA class and cardiac area on the chest radiographi .However, despite their tendency to the rise when there is involvement of the systolic function, there was no correlation with ejection fraction in echocardiogram. The presence of a pacemaker and the changes found in the ECG did not PNB levels, either. The individuals seropositive for DC, who are asymptomatic and with no ventricular dysfunction have circulating levels of PNB similar to those of healthy people
