RESUMO

The classifications of third lower molars observed in the literature commonly valorize the angulation of the teeth to orient the surgical technic. However, there is a lack of articles talking about previous establishment of surgical difficulties. The aim of the present study was to elaborate a radiographic classification related to the surgical difficulties of impacted mandibular third molars based in panoramic tomographs. There were selected from the review of the literature, the 21 frequently cited radiological variables. Applying selection criterias, only ten variables were included to analysis and mensuration in 303 panoramic tomographs of patients submitted to surgical removal. The 303 surgical plannings and surgical techniques utilized were analyzed by means of relatories. The data were submitted to statistical analysis with the objective of verify the variables more related with the surgical difficulty (Kruskal-Wallis test) and to reduce the number of variables containing similar informations (Multivariate analysis). According the results, four variables were selected: CS (relation between third and second lower molars); OD (distal bone); PT (lower third molar depth) and PR (root pattern), which compose the acronym COPP. The classification was based in the number of unfavorable variables, as follows: N1 (when no more than one variable was unfavorable); N2 (when two variables were unfavorable) and N3 (when three or four variables were unfavorable). The proposed classification was applied in 303 radiographs (in a retrospective study) to comprove its clinical validity. The results demonstrated the confiability of the classification (97,69%), where the four variables (PT, OD, PR and CS, in decreased order of importance) adequately classified the level of surgical difficulty of included lower third molars.
