Causas do abandono do tratamento da tuberculose no Centro de Doenças infecto-parasitárias e no Centro Especializado Municipal, Unidades de Referencia de Campo Grande, MS, 2002 e 2003

RESUMO

This descriptive epidemiological study of a series of cases characterizes those of tuberculosis treatment abandonment in the reference Health Unitsthe Centre for Infectious and Parasitic Diseases (CEDP) and the Specialized Municipal Center-in Campo Grande, Mato Grosso do Sul State, Brazil, during 2002 and 2003. Data were collected in a study of the registers of the National System of Notifiable Diseases of the Municipal Public Health Secretariat, followed by an interview at the home of the cases registered, after the signing of a Document of Informed Consent. The form used contained the following variables: treatment and promptness of the user, access to the treatment unit, side-effects of the medication, eating habits, other diseases, vices, professional activities, limitations due to the infection and motivation for abandoning treatment. Results: 195 cases were identified in these units (107 in 2002 and 88 in 2003), of the total of 514 notifications in the Municipality. The following results were seen: 153 cases cured (78.4%), 10 abandoned (5.1%) 16 cases transferred (8.2%), 9 deaths (4.6%) and, in 7 cases (3.5%), change of diagnosis. Amongst the 10 cases of abandonment, 7 were in 2002 and 3 in 2003: two condemned criminals of unknown abode; 2 tramps; 3 moved without leaving new addresses; one addict (24 years old), Afro-Brazilian, of unknown schooling, new pulmonary case, abandoned treatment at three months because of dizziness, uses public transport to get to the treatment unit, alimentation adequate, AIDS as an associated disease, drinks alcoholic beverages (distilled and fermented), actual occupation guarding cars, interrupted treatment feeling better and cured; one prisoner (29 years) in the Municipality of Dourados, mulatto, 4- 7 years of schooling, new case with tuberculous meningitis, 4 months of treatment but feeling ill, treatment abandoned because of the need of police escort to the treatment unit, alimentation adequate, AIDS as an associated disease, smokes one packet of cigarettes daily; and a housewife (59 years), Afro-Brazilian, no schooling, new case with tuberculous meningitis and 3 months of treatment, uses public transport to reach the treatment unit, accepts the medication but suffers from abdominal pain, headaches and apathy, interrupted treatment because she believes in a curse on her. The three interviews cases are from the CEDIP in 2002. In the units studied, the following results were identified: 3.4% of abandonment was seen in 2003, below the 5% limit set by the Ministry of Health; the individuals with predisposition for abandonment, both socially and due to their living conditions; the possibility of monitoring; improvement in the system of vigilance and the flow of information; maintenance of the sensibility of activity of the local health team; treatment and control of cases with direct supervision, including evaluation of contacts, health education, provision of medication and the organization of service activities with the support of the Program of Community Agents and Family Health (PACS-PSF), remaining efficient and dynamic in collaboration with other municipal health units and in Mato Grosso do Sul State.
