Análise da interação entre o odontopediatra e a criança em situação de atendimento odontológico

RESUMO

The literature has been demonstrating that the pediatric dentistry treatment is influenced by the children?s and professional?s pattern of behavior. However, it is necessary to carry through more studies to investigate some variables of this context. This research intended to characterize the pattern of interaction among pediatric dentistry and the children, during prophylaxis or emergencies attendances. Two pediatric dentists were requested to take part on the research. Twenty children, which were considered non clinical for behavior problems according to the results of the Walker Problem Behavior Identification Checklist filled by their mothers, also participated. The data were collected in a clinic-school of pediatric dentistry, through the writing in video of the attendances. Each professional attended ten children, five of then during prophylaxis and five during emergency. The observed responses were categorized and analyzed through an adaptation of a categorization system which was used in a previous study. The categories established for the children?s behavior were: cooperative and opponent; for the pediatric dentist?s behavior they were: adequate (positive handling strategies) and inadequate (punitive or restrictive handling strategies). The obtained results indicated that 19, from the 20 children which were attended, showed high frequency of cooperative responses, and that eight showed high frequency of opponent responses, which means that the non-collaboration frequency was high in 40% of the total of attendances. The children attended during emergency showed 50% less cooperative responses and two times more opponent responses, comparing with the children attended in prophylaxis. Concerning the pediatric dentists, it was observed a high frequency of adequate responses, by minute, than of inadequate responses, in all of the ten prophylaxis attendances, but only in four of the ten emergency attendances. There was an elevated frequency of the professional?s inadequate behaviors in relation to children which showed high or low frequency of opponent responses. The data suggest that: a) children were less cooperative and more opponent during the emergency attendances than during prophylaxis attendances; b) the occurrence of children?s opponents responses seems not to exert great influence on the occurrence or not, of the professionals inadequate responses; and c) the variable type of attendance exert influence on the occurrence of professionals adequate and inadequate behaviors, with more difficulties on the handling of the children during emergency attendances. These conclusions are evidences of the necessity of implementing the abilities of the pediatric dentists which are demanded on the emergency attendances. It has to be done aiming a decreasing on the usage of inadequate handling strategies during this type of attendance. So, it is suggested the measures should be taken on the courses of graduation and post-graduation in Dentistry and Pediatric Dentistry, to implementing the training on the technical abilities of the professional, and on the positive abilities of children?s behavior handling.
