RESUMO
The objective of this research was to describe and to evaluate the process of decentralization of the Workers Health Surveillance (WHS) in the Brazilian National Unified Healthcare System (SUS) at the City of Campinas. The study was conducted as a formative evaluation research, with a mixed professionals assessment team in multicentric settings that included the Central Coordination of Health Surveillance (Covisa); the Workers Health Reference Center (Cerest); all five districtal teams of Health Surveillance (Visas); the 49 health centers of the city and State University of Campinas (Unicamp). A triangulation method was deployed to achieve an embracing analytical approach combining quantitative, qualitative elements, in historical perspective of the political context and the relationships amongst different social actors involved. From the established directives of the decentralization identified through official documents issued by the City Health Department of Campinas we evaluated the decentralization process by focus groups interviews within the teams of each Visa, Cerest, and Covisa and semi-structured interviews with health surveillance professionals of each health unit. The indicator of results was a combination of: the number of workers accidents and illnesses registered at the health centers; the proportion of those workers related with the formal or informal job market; and if those work related health problems generated surveillance actions of health interventions, that could promote changes in the job processes and the general environment. Results obtained show aspects of an effective decentralization process like health care providing to workers in the health centers, the improvement of Visas capacity and the integration with environmental health surveillance. However, important questions about the health services use and the health information systems still remain, mainly with regards to the explicitness of the functions and relationships among different levels of the Municipal System for WHS
