RESUMO

Introduction: The decentralization of Health surveillance, although catapulted from the elaboration of the Adjustments and Goals Term, cannot count on commonsense criteria and standards that could be used by the States when it comes to evaluate the decentralization itself. Objective: To create and validate a framework for the Health surveillance in the city administration that can correspond to the definition of what a City System of Health surveillance adequate to the needs of the population. Methodology: A development study was carried out according to the typology of Contandriopoulos. For the creation of a framework of the decentralization of Health surveillance a logical model was elaborated from which a matrix derived containing dimensions, criteria and indicators capable of evaluate the structure, the administration and the results of the Local System of Health surveillance. The choice of criteria and indicators was supported by an analysis of the literature, the summary of documents and relevant legislation. After that, this proposal was subjected to a group of nine experts so that they could validate the process through a technique called Consensus Conference. Results: Among the proposed criteria 59% presented a consensus whereas 41% didn?t. Only one was considered as having little importance whereas 53 were considered to be important. That is what granted us with the authorization to consider the matrix proposed as validated by the experts consulted. The obtained consensus besides being able to be used for evaluation can also is a reference for the organization and the improvement of City Systems of Health surveillance. The main components of these systems would be: adequate infrastructure and financial autonomy; sector and inter-sector planning, a policy of human resources as well as of instruments to guarantee the standardization and quality of the process; integration among the different watching organizations and the implementing of communication actions and informative activities; the implementation of programs for the increasing of the awareness of the population regarding sanitation and healthy professionals among others. The author discusses the temporary essence of the obtained consensus as well as the meaning and possible uses of the final framework emphasizing the very need for it to be temporary and the fact that it represents only a starting and reference point for a process of redefinition of criteria and of the renewing of the consensus.
