Videolaringoscopia em voluntÃrios com esquistossomose mansÃnica na forma hepatoesplÃnica

RESUMO

It was investigated the possible alterations in the larynx in bearers of hepatosplenic schistosomiasis mansoni (HSM). Sixty five volunteers were divided in three groups. Twenty-four bearers of HSM (SG-I) twenty-one bearers undergone splenectomy (SG-II), and twenty, of age and similar social-economic-geographic conditions, without the disease (CG). Besides the otolaryngologic examination, all the volunteers underwent videolaringoscopy. The age of three groups were similars: SG-I 57.9 Â 11.9 years; SG-II 53.1 Â 9.6 years and CG 54.9 Â 8.8 years â p>0.05. The following videolaryngoscopic findings were observed between the SG-I vs CG; SG-II vs CG and SG-I vs SG-II): hyperemia and edema in the mucosa of the arytenoids and posterior comissure (96.0% vs 45.0% - χ2 = 11.76 - p = 0.00016; 71.4 % vs 45.0% - χ2 = 1.96 â p = 0.1615 e 96.0% vs 71.4% - p = 0.0388), bowing anterior-posterior glottic chink (50.0% vs 45.0% - χ2 = 0.0000 p = 0.9780; 28.6% vs 45.0% - χ2 = 0.59 - p = 0.4428 and 50.0% vs 28.6% - χ2 = 1.34 - p = 0.2465), edema in true vocal cord (TVC) ( 87.0% vs 55.0% - χ2 = 4.29 - p = 0.003841; 28.6% vs 55.0% - χ2 = 1.96 - p = 0.16155 and 87.0% vs 28.6% - χ2 = 13.84 - p = 0.00019) and hiperemia in TVC (66.7% vs 45.0% - χ2 = 1.30 - p = 0.2546; 28.6% vs 45.0% - χ2 = 0.59 - p = 0.4428 and 66.7 % vs 28.6% - χ2 = 5.07 - p = 0.0243). The frequencies of the hyperemia and edema in the mucosa of the arytenoids and posterior comissure were significantilly greater in the groups SG-I and SG-II. There was a trend for these inflammatories manifestations being more frequent in SG-I as compared to SG-II, sugesting that the HSM can produce atipical gastroesophageal reflux with reduction in the frequency in the SG-II aproaching of the frequencies of the CG. The frequencies of bowing anterior-posterior glottic chink were not statistically different between the groups. The frequencies of the hyperemia and edema of the TVC were significantilly greater in the SG-I and SG-II, when compared with CG. It can be concluded that in bearers of HSM there is a greater prevalence of alterations of the larynx, mainly hyperemia and edema of the arytenoids and posterior commissure, associated gastroesophageal reflux (GER). This condition is probably related to the increase of the intra-abdominal pressure due to increased liver and spleen. It was observed greater prevalence of the hyperemia and edema in the TVC with the same association of the GER, product of the increase of the intra-abdominal pressure due to increased liver and spleen. It was verified that, the frequency of GER atipical reducted after splenectomy, being evidence that the increase of intra-abdominal pressure, can interfere in your genesis. It was not possible to verify the patogenic association between HSM, per si, and the larynx alterations
