Variabilidade da freqüência cardíaca no domínio da freqüência, durante o sono, em crianças e adolescentes portadores de anemia falciforme

RESUMO

Objectives To analyze the autonomic nervous system behavior, through HRV, during sleep, in children and adolescents with sickle cell disease (SCD). Methods - Forty four SCD patients, mean age 13.7 were submitted to polysomnography and Holter monitoring to assess HRV during the night. Both equipments were synchronized, to allow for simultaneous analyzes. The total spectrum power, low frequency (LF), high frequency (HF) component and LF/HF ratio, LF, HF normalized units were evaluated wakefulness, stage 4 and REM sleep. Oxygen saturation and hemoglobin level were check out and correlated with the HRV. Result The individual sleep stages differed in total spectrum power. Its values were significantly higher in the REM stage than in stage 4 (p=0,005).There was a significant increase in HF (nu) at stage 4 when compared to wakefulness (p=0,019). The LF/HF ratio did not reach statistical significance between stages of sleep, but there was an uncommon decrease at the REM stage. The LF and HF components reached their highest values at REM stage, and LF (nu) was kept elevated in all stages of sleep, but without statistic significance. Low level of oxygen saturation was present in all sleep stages (mean at wakefulness-87.76, stage 2- 87.41, stage4 - 86.52, REM - 87.85), without significance between stages of sleep and HRV. Conclusion SCD patient shown predominance of sympathetic activity in all stages of sleep, and loss of regulation capacity of the autonomic nervous system. The shift in xiv spectral analyze, which occurs at different sleep stages in health subjects, did not occur in patient with SCD.
