RESUMO

New clinic interventions in heath care, soon acquire a mythic aura, by the argument that they are modern and better. The risks are minimized and the products become social necessities for consumers and physicians. In case of mental health, where diseases are diagnostic constructs, this phenomenon has expanded. Many studies show that people with severe mental disorders are undertreated while people with random symptoms that not constitute a psychiatry disease receive an iatrogenic treatment. Thus, the use of psychotropic affects not necessarily the public who would have a real benefit. The objective of this project is comparing the use of psychotropic at health units with and without clinical expanded arrangements. Furthermore the objective is also analyzing differences of this use in the services dynamics. The health units from low socioeconomic index zones of Campinas-SP was categorized on the presence or absence of clinical expanded arrangements (existence of team meetings, management care case; registration of customers; good relationship between general practices and experts; democratic local administration; groups education, promotion and home care) by the cluster technique. We have quantified the prescription of psychotropic. Moreover, we ve done focus groups and individual interviews with health workers and patients, to inquire them about the psychiatry medicines role on theirs practices. We ve observed high rates of drugs prescription among the population in both groups: 8 to 10% of antidepressants, and 7.5% of benzodiazepines. The clinical expanded arrangements effectiveness has a greater impact on the production of critical workers, but this effect wasn t seen on patients. There wasn t an open dialogue about the medicines, and its uses between health workers and patients. Patients are used to seek information about their treatment in pharmaceutical leaflets, since it does not feel supported by the health teams. Only psychiatrists are responsible for psychotropic prescription in Campinas. Other team members such as general practices and nurses don t feel responsible for this. In attempts to stop their medicine, mainly due to side effects, patients do it without proper medical support. Then, they suffer with the withdraw syndrome, which reinforces the fear to question the medical power. Despite of this, they valorize alternative approaches to their problems and criticize the lack of marketing on these interventions.
