ÚLCERA POR PRESSÃO EM IDOSOSHOSPITALIZADOS: análise da prevalência e fatores de risco .

RESUMO

The fidgets had appeared from acquaintance with aged patients during the practical professional in hospitals, home-care, having folloied familiar and friends. It was perceived necessity of the knowledge extended by means of the accomplishment of studies that disclose, the prevalence of the ulcer for pressure of hospitalized aged people, for a bigger knowledge of the reality of these patients and the related factors of risk to the ulcer for pressure, with the attention directed for the prevention of this type of injury. The objective of the research was to analyze the prevalence and factors of risk of the ulcers for pressure in aged hospitalized. The sample was composed of 300 manuals of elders interned public hospital of tertiary level in Fortaleza, between 2003 and 2006. The study was approved by the Committee of Ethics in Research of the Institution. The data were collected using a form that contemplated all the important annotations concerning to the thematic. The majority of the population studied was of elders in the age group of 73 to 83 years (34,7%). The pathologies most found in the elderly investigated by the research were the CVA (60%) and the arterial hypertension (74,3%). Of the 300 elders, 27(9%) were smokers and 10(3,3%) were alcoholics. Regarding to the surgeries, 33,3% of the patients carried out some procedure. The medicines more associated to the occurrence of ulcers during the hospitalization were antinflammatories, bronchodilatation, use of vasopressors, dopaminergics(20,7%) and antibiotics(12,3%).The risk factors more associated to the development of the wounds were the chronic-degenerative pathologies, immobility in the bed, others (coma, medicines), deficit in the nutrition state and pressure. Among the prevention conducts registered in the manuals we detached the absence of any conduct (86%) and change of decubitus (12,3%). The conducts of handling more utilized were the curative without specification of the kind (5,6%), colagenase (4%), absence of conducts registered in the manual (3%) and curative with hydro colloids (3%). The predominance of ulcers by pressure found in 2003 was of 23,2%, in 2004 (11,1%), 2005 (19,9%) and in 2006 of 21,3% during the hospitalization. Recommendations for the prevention of the ulcers for pressure: elaboration of one program of prevention of the ulcers for pressure, to make use of professional nurses specialists in wounds (enterostomal therapists) with the purpose to implement protocols of assistance and evaluation of the carrier of wounds and to use scales of prediction of risk for development of the ulcers for pressure.
