Teste ergomÃtrico em crianÃas e adolescentes : comparaÃÃo entre os protocolos de Bruce e rampa

RESUMO

The exercise test (ET) has been performed in children and adolescents to assess exercise capacity, before the start of physical activities, in the presence of heart disease or to evaluate symptoms related to effort. The treadmill has been the most frequently used ergometer associated with the Bruce protocol. There has been increased interest in individualized protocols, lasting between 8 and 12 min, with small constant increases in speed and inclination, in accordance with the predicted VO2 max, known as the ramp protocol. The purpose of this study is to compare the duration of effort, heart hate (HR), speed, grade and VO2 max of the young people submitted to one of these protocols and to suggest approaches for the use of the ramp protocol. A series of cases type observational study was performed on 1006 children and adolescents aged between 4 and 17 years, who had been submitted to the ET from October 1986 to February 2003 and completed one of the two protocols. Those that were excluded had the ET interrupted for causes other than physical fatigue, use of medications that interfered with HR and physical limitations preventing regular exercise. The statistical analysis of the data was done adopting a significance level of p <0.05 and a confidence level of 95%. The duration of the exercise of around 10:00 min, was practically equal among the girls, while with the boys it was significantly longer with the Bruce protocol, with values over 12:00 min from the age of 12. The max HR achieved was over 180 bpm in the two protocols at all ages. The inclination was a little higher in the youngest girls with the Bruce protocol, and the speed and the VO2 max were greater in all individuals with whom the ramp protocol was used. The VO2 max, the speed and grade achieved can be used as baseline when the ramp protocol is chosen using the treadmill
