RESUMO

Introduction: Obesity represents an emerging public health problem among adolescents, usually attributed to inadequate food intake and changes in lifestyle, associated to genetic predisposition. However, other mechanisms like socio-economic background, emotional and behavioral difficulties are also related to overweight. Emotional and behavioral problems may be associated with excessive gain of weight in adolescence by direct action on components of neuroendocrine axis, but questions still exist in this area, especially in the role of protective factors such as resilience. Objectives: To estimate the prevalence of behavioral problems among adolescent in a mixed urban-rural region, according to gender, age and ethnicity and investigate the relationship between behavioral problems and overweight, studying the role of gender and resilience of the adolescent on this relationship. Methods: The thesis consists of three separate papers, which have the emotional and behavioral problems (EBP) as common features. These were identified by the use of the Youth Self Report/11-18 (YSR/11-18). Data were obtained in a cross-sectional study through the school census (960 adolescents) from an urban-rural mixed area, representative of the adolescent students of the region. The first paper describes the prevalence of emotional and behavioral problems among adolescents in an area of rural-urban transition, with emphasis on differences between genders. Prevalence ratios (PR) were adjusted for age, skin color and grade level. The second paper, using exploratory factor analysis, describes the factorial structure of YSR/11-18, related to the eight originally described syndromes. The third paper investigated the association between behavior problems and overweight/obesity, evaluating the role of gender and resilience in this association. The Resilience Scale (RS) was used to measure resilience. The association was investigated using Poisson multivariate regression models. Results: Paper 1 - Higher scores for YSR/11-18 behavior problems were found among girls, except for the scale rule-breaking behavior. The prevalence of EBP was higher among girls (PR: 1.61; 95%CI:1.062.46), who also had more social problems (PR: 2.88; 95%CI: 1.515.51). Paper 2 - Grouping of the scale items "attention problems" was not obtained. Five items of the scale "social problems formed a single factor with items from the scales anxious/ depressed and withdrawn/depressed, suggesting a dimension of internalizing problems. The externalizing dimension grouped into two factors, moving away from the original characteristics of the scales "aggressive behavior" and "rule-breaking behavior". On the other hand, for somatic complaints and thought problems there was correspondence with the X original scale. Paper 3 We found a positive association between total behavior problems and overweight after adjusting for potential confounders (PR: 1.74; 95%CI 1.202.52). The gender functioned as the effect modifier of this relationship, observing this association only among girls (PR: 1.90; 95%CI: 1.282.84), a finding also found for internalizing (RP: 1.60 95%CI: 1.112.32) and externalizing (PR: 1.62; 95%CI: 1.032.55) problems. The strength of association between total behavior problems and overweight was 65% higher among youth with low resilience. Conclusion: Behavioral problems were relevant among adolescents from a non-urban area and were associated with the presence of overweight. The observer effect modification for gender and resilience warns for directing preventive and treatment actions in public health.
