Prevalência de benefícios temporários por doença respiratória no Brasil, 2003-2004

INTRODUCTION: Respiratory diseases are major cause of disability for work. However the existing epidemiological data are vague and inconsistent. To evaluate the database of the National Institute of Social Security (INSS) has become a great opportunity to better know the clinical, occupational and social security impact caused by this group of diseases. OBJECTIVE: To determine the prevalence of the temporary benefits by respiratory disease in workers employed in Brazil for the period 2003 to 2004, seeking to identify the potential relationship between the branches of economic activity and this group of disease, and the days of work lost and the cost arising from these disorders. METHODS: This is a cross-sectional epidemiological study that examined the temporary benefits granted by the INSS to employees. We evaluated sex, age, diagnosis, branch of economic activity, type, duration and cost of benefits. The results were presented in the form of coefficient and rate of prevalence, average and standard deviation of the duration and cost of benefits. The data were collected from the benefits of the Unified System Benefits of the INSS. The population data were obtained from the National Register of Social Information, and refer to the links declared in the period. RESULTS: Respiratory diseases accounted for 1.3% of total temporary benefits granted by the INSS, with a coefficient of prevalence of 9.92 (per 10,000 links). Women were the most affected, with a prevalence rate between women and men of 1,14:1,00. The age groups above 50 years had the highest prevalence of respiratory disease (57.40). The not work related benefits predominated in relation to work related (ratio of 34,34:1,00). The most prevalent diseases were pneumonia (1.77), asthma (1.69), chronic obstructive pulmonary disease (1.12) and diseases of the vocal chord and larynx (0.86). On the branch of economic activity, the highest prevalence occurred in the manufacture of other transport equipment (33.95), manufacture of tobacco products (31.34) and computer and related activities (20.25), and the last showed higher characterization of the benefits as related to work (3,80:1,00). The total work days lost was 5,157,537, with a total cost of R$ 110.570.836,97. The median and average duration was 106 and 209.68 days, with median and average cost of R$ 1.993,70 and R$ 4.495,30 for benefit. Respiratory diseases by exogenous agents demanded greater duration (median of the 248 days and average of the 296.72 days) and cost (median of the R$ 4.336,42 and average of the R$7.105,74). CONCLUSION: The most prevalent diseases were the airways diseases and pneumonia. Workers from the manufacture of other transportation equipment, manufacture of tobacco products and computer and related activities were the most disabled. The diseases by exogenous agents demanded greater cost and duration. The characterization of the technical link with work was poorly established.
