Fatores associados à realizaÃÃo de prÃ-natal nÃo adequado : um estudo das puÃrperas atendidas em dois hospitais de referÃncia para gravidez de alto risco do SUS em Recife

RESUMO

A transversal study was performed, with the purpose to investigate what are the factors that influence in the inadequate prenatal care in mothers after delivery who are attended in the public sector of health in Recife, Pernambuco, Brazil. A total of 612 women were interviewed in the immediate post-partum, from 10th June to 10th October of 2004, at the Hospital Agamenon MagalhÃes and Hospital BarÃo de Lucena, which are public health services from the Brazilian Public Health System (SUS- Sistema Ãnico de saÃde) and also references to high risk pregnant women in the state of Pernambuco. The women receiving inadequate prenatal care was characterized according to some variables such as: socioeconomic condition, reproductive history from the mother, pregnancy risk, kind of service and satisfaction that women with the prenatal attendance. Data were collected from interviews with a structured and codify questionnaire or copied from the medical records by a well-trained group of interviewers. It was observed a prenatal coverage of 94,4%, whereas the average of visits was 5,3 and a percentage of 35,2% of the women had six or more prenatal visits. However, in spite of this coverage, a high proportion of women received inadequate prenatal care (69,8%) and didnât guarantee the quality of the care performed. According to the results of the multinomial logistic regression some factors were identified as influence in the realization of the inadequate prenatal care, such as: school life lower than forth degree of elementary school ((OR=4,54; IC95% 1,06-19,53), low per capita family income (OR=1,87; IC95% 1,18-2,93), none desire for pregnancy (OR=1,53; IC95% 0,99- 2,39), absence of next attendance in that prenatal care service (OR=2,10; IC95% 1,12-3,96), besides the high and intermediate pregnancy risk (OR=1,66; IC95% 1,05-2,62). The occurrence of the dead born child in a previous delivery (OR= 0,38; IC95% 0,14-0,99) is a protector factor to the realization of the inadequate prenatal care. According to the results, itâs possible to conclude that, even though the prenatal care offered to high risk pregnant women at the two public hospitals in Recife, PE have shown a good coverage, some effort is still necessary to improve the quality of attendance
