EMISSÕES OTOACÚSTICAS EVOCADAS POR ESTÍMULO TRANSIENTE EM CRIANÇAS SUBMETIDAS À PALATOPLASTIA

RESUMO

INTRODUCTION: It is commonly observed that situations in which the mere suspicion of individuals with middle ear (ME) alterations is reason enough to get them automatically excluded from the sample about investigation of evoked otoacoustic emissions, even when it is about population that is frequently taken with conductive problems as it is the childrens case. Therefore, it is little known about records of otoacoustic emissions in individuals with alterations of ME. AIM: To verify the influence of alterations on middle ear in Transient Evoked Otoacoustic Emissions ( TEOA) on children submitted to palatoplasty between the age of 4 to 12, being 24 male and 10 female. The children were submitted to the visual inspection of the external ear canal, questionnaire, threshold tone audiometry, acoustic impedance and TEOA. For the analysis of TEOA, two criteria were use. One of them was proposed by Prieve et al. (1992) and the other based on clinical criterion in which the presence of response of TEOA was analyzed by bands of specific frequencies. RESULTS: When the analysis of TEOA was accomplished according to Prieve et al. (1992), a larger absence of occurrence was observed when compared to the criterion of analysis for frequency band. The analysis of records of TEOA for frequency band aids us in the supposition of the prognostic of the alteration is smaller, EOET can appear in more bands of frequencies. However the analysis of TEOA for bands of frequencies should not be used separately but in a combined way, integrating information about the anemnesis with the data of audiologic exams. Its important to observe that the records of DPOAE are fundamental. Regarding the EOET results with threshold tone audiometry, it was observed in increasing number of ears with gap and EOET became more and more difficult to be assessed. Concerning tympanometry and both criteria used for analysis of TEOA, it was noticed a tendency of presence of TEOA associated to the tympanometry of type A and absence of TEOA in tympanometry of type B. Though, inconsistency was verified among the findings of TEOA associated to the tympanometry of type C. About the acoustic reflex (AR), it was noticed that with the worsening and/or absence of AR, the chance of records of ATEOA was smaller. CONCLUSION: It was noticed that the history of ME disturbance can lead to absence in the records of TEOA, even with normal threshold tone audiometry and tympanometry of type A. Despite of the fact that TEOA is a valuable clinical procedure, its use un a isolated case doesnt offer considerable conclusion for auditory evaluation, being necessary the accomplishment of other behavioral and objective ones in order to obtain the logical auditory diagnosis.
