DemÃncia associada ao HIV em idosos: estudo seccional na cidade do Recife, Brasil e revisÃo de literatura

RESUMO

The human immunodeficiency virus (HIV) is a retrovirus which infection present as a pandemic. Pathogenically, is responsible for a depression of the immunological system mainly by means of depletion of lymphocytes T CD4 and, then, the opportunistical infections that follow. The virus, beyond immunodepression, is associated with a direct lesion of some organs, as the central nervous system, and, owing to that involvement, the HIV-associated dementia. After the introduction of the antiretroviral therapy at the end of the last century, it has been perceived a rise in survival among infected ones. Associating this to advances on diagnosis and changes in behavioral patterns, it is been noted an aging of the HIV infected population. The present study consists of two articles. The first is a review article, that in addition to address the changes in HIV-associated dementia presentation after the institution of the antiretroviral therapy and aging of the infected population, evaluate the most recent evidences about the interaction between aging and the dementia at the HIV infected patients. It has been perceived a rise on the prevalence of dementia in those older than 50 years old infected, besides the interaction of HIV with other kinds of cognitive impairment as Alzheimer disease, for example. Approach the changes on diagnostic evaluation, with newer and more practical evaluation scales, and treatment, antiretroviral or neuroprotective strategies, either and the best form of insert those on this new aging population reality. The second is an original article, in the form of a sectional study, that aimed to describe the frequency and risk factors to neurobehavioral changes in older patients seen at two reference centers in the assistance of the HIV infected people in the city of Recife, at Brazil northeast. In the study, was evaluated the frequencies of cognitive impairment and dementia, just as depressive symptoms, in patients with 50 or more years old infected with HIV in an outpatient setting. It was identified a high frequency of that disturbs with more than a third of the evaluated patients presenting cognitive impairment and depressive symptoms, with frequencies of 36.5% and 34.6% respectively. Identified too a higher frequency of cortical involvement in those patients with cognitive impairment. Considering the two articles, a new perspective about the comprehension of the cognitive impairment of an older HIV population is presented
