Compreensão ritmica dos membros : uma nova tecnica para o tratamento das ulceras isquemicas

In this study our objective was to evaluate the efficacy of rhythmic limb compression (RLC ) as an auxiliary measure to aid in the healing of ischemic limb ulcers. Initially, the applicability of the method was tested by examining the efficiency of trans-vessel pressure transmission using the radial artery of five patients with normal vascular arteries and the popliteal artery of five individuals with atherosc1erosis. On average, 97,7 % and 86,7 % of the pressure applied to the radial and popliteal arteries, respectively, was transmitted to the interior of the vessels. Subsequent1y, the method was applied to 30 patients with u1cers in their limb extremities. Of these, 15 suffered from systemic sc1erosis (Ssc) with a total of 24 ulcers and 15 had atherosc1erosis with a total of 15 ulcers. Following 20 one-hour sessions, healing was observed in .62,5 % of the Ssc patients and in 73,3 % of those with atherosc1erosis. All sessions were carried out on an ambulatorial basis, three times a week. The pain associated with the ulcers disappeared between the fifth and fifteenth sessions in those patients who showed healing. All of the patients were followed up for 30, 60 and 90 days after terminating RLC treatment. In only two cases, both involving Ssc patients, was there a recurrence of the ulcers after 30 days. One patient in the atherosc1erosis group whose u1cers did not heal underwent an amputation within 30 days after ending RLC therapy while another with a healed ulcer died following a heart attack 60 to 90 days after terminating treatment. In general, those ulcers which had healed by 30 days post-treatment remained so for the remainder of the follow-up period while those which had not healed by 30 days post-treatment also did not heal by the end of the follow-up. It is concluded that the treatment applied accelerated the healing of ischemic ulcers. No complication associated with the use of this approach was observed
