Avaliação muscular e capacidade de exercício em pacientes com doença pulmonar obstrutiva crônica leve/moderado com normoxemia ou hipoxemia leve diurna e dessaturação noturna sem síndrome da apnéia obstrutiva do sono

RESUMO

We carried out a transversal study which included 21 patients with mild/moderate COPD, divided into a nocturnal dessaturator and a nocturnal non-dessaturator group without OSA. We first studied the total sample, to characterize its respiratory functional components, sleep and exercise. We analyzed the pulmonary function, polissonography variables, peripheral strength and respiratory muscular strength of the groups. There were no differences concerning exercise capacity, peripheral muscular strength and respiratory muscular strength among the groups. There was a strong positive statistical correlation between the predicted PImax % and mean nocturnal saturation in the desaturator group. The ventilatory, metabolic and hemodynamic adjustments were also alike (p>0,05 for all groups), although the dessaturator group featured a DAPmax adjustment connected to mean nocturnal dessaturation (p<0,0001). The number of patients with above normal CRP statistically differed among groups (p<0,05). There were more patients with this characteristic in the dessaturator group. The dessaturator group presented statistically lower serum hemoglobin levels if compared to the non-dessaturator group (p<0,05). The evidences found in this study favor low level systemic inflammation caused by nocturnal dessaturation (diurnal Pa02 regardless) but the exercise capacity in cycloergometry was not significantly affected by mild nocturnal dessaturation.
