RESUMO

Objective: The goal of this research was to describe the peripherical and central audiological findings of a child with auditory neuropathy/auditory dys-synchrony. Methods: It was performed an audiological evaluation, eletrophysiological and behavioral, with tests that appraise the peripherical and central auditory system. Results: The result found in audiometry revealed a variation or floating of pure tone thresholds and the beginning of a sensorioneural hearing loss in high frequencies; the results of speech audiometry were compatible with pure tone thresholds. It could be observed a presence of the contralateral acoustic reflex in the frequency of 500 Hz in both ears and 1 kHz on the left ear, with absence of reflexes in the other frequencies, with normal timpanometry. The otoacoustic emissions were not found, when compared to previous evaluations, it can be observed that a hearing loss is occuring comparing the results from previous exams. The ABR register showed absence of response with presence of cochlear microphonics, in the minimum level of 50 dBnHL on the right ear and 60 dBnHL on the left ear. The auditory evoked potentials for middle and long latency were presents. In the evaluation of the auditory processing we can find normal results for the following tests: low-pass filtered speech, auditory fusion test, dichotic digits and pitch pattern sequence test; the results for the following tests showed altered: speech in noise test, synthetic sentence identification, staggered spondaic words, masking level difference and random gap detection. Conclusions: The use of all of the audiological exams, eletrophysiological and behavioral, in order to evaluate the peripherical and central auditory system is fundamental in the cases with auditory neuropathy/auditory dys-synchrony, once they can make possible a better comprehensions of these cases; its important to follow up them carefully even when normal thresholds can be observed because light modifications can occur which may need a therapeutic intervation
