Aplicabilidade de parâmetros antropométricos e de bioimpedância elétrica na avaliação do estado nutricional de idosos / Applicability of anthropometric parameters and bioelectrical impedance in the nutritional status assessment of elderly

RESUMO

This study aimed to evaluate the applicability of phase angle (PA) and bioelectrical impedance vector analysis (BIVA) in the nutritional status assessment of elderly. There was an epidemiological study of cross design, in which 70 elderly individuals aged 60 to 98 years old (n=33 men; n=37 women) assisted by Family Health Program of the municipality Viçosa-MG were evaluated. To selection of the sample, it was made a screening in the thirteen units of Family Health Program. Weight, stature, waist circumference (WC), calf circumference (CC), knee height, body mass index (BMI) and informations about diseases and medicines consumption were obtained of the 864 volunteers of both sexes. Of these, were included in the sample 33 men and 37 women considered healthy or with hypertension controlled through medication, with BMI between 22 to 27 kg/m2. The anthropometric indicators evaluated were: weight, stature, BMI, CC, arm circumference (AC), arm muscle circumference (AMC), triceps (TST), Bicipital (BST), subscapular (SST) and suprailiac (SIST) skinfold thickness. The fat free mass (FFM), fat mass (FM) and percentage fat mass (%FM) were calculated. Resistence (R) and reactance (Xc) from bioelectrical impedance (BIA) were used to calculate PA. The BIVA was applied. Statistical analysis consisted of the coefficient correlation of Pearson and Spearman, the Student t test, Mann-Whitney, Hottelings T2 test and the distance of the vectors of the impedance was calculated by Mahalanobis D distance. The biochemical analysis consisted of complete blood cell count and serum albumin concentration. The results presented that, although both sexes showed similars BMI, the body composition was different. The values for PA and muscle mass indicators were higher in the male group and fat mass indicators, were higher in the female group. A reduction trend of the muscle mass and fat mass indicators was observed in both sexes with advancing age, excepted of FFM in men. Women >70 y showed significant minor values of PA. The PA had positive association with FFM in men and negative association %FM in men and women >70 y. There was a significant displacement of the mean womens impedance vector to less mass and structure in RXc graph compared with mens vector. It concluded that the PA and BIVA are good indicators in the determination of the nutritional status of elderly individuals, a time that they were capable to detect the differences in the body composition quality and the modifications of the advancing age. Because of the simplicity and non-invasiveness, it is suggested the use of BIA in nutritional surveillance programs, geriatric routine and clinical practice in aged.
