RESUMO

It has been a controversial subject the choice of surgical technique in low rectum anastomosis. The use of stapler has made it possible to extend the low anterior resection below the peritoneal reflex ion, preserving the whole sphincter mechanism. However the real value of this surgical methodology has not been clearly settled yet. This comparative randomized study between the stapling and the handsewn suture in the rectum of dogs below the peritoneal reflexion led to this research. Twenty dogs underwent proctocolectomy and the restablishment of the gastro-intestinal transit was made in two different ways: a group of 10 dogs by manual sutura and the remaining group by mechanical suture. Both groups have been evaluated on the fifth and fifteenth days of the post-operative with special attention to the concentration of the collagen in the suture line, to the macro and microscopic aspects and to the complications of the anastomosis. This analys is led to the following conclusions: (1) there was no significant difference between bothanastomosis through macroscopic evaluation; (2) on the fifth day of the post-operative, the group with the mechanical suture showed a better healing and a bigger concentration of the collagen in the area of the anastomosis; (3) amore intense stenosis of the anastomosis was found in the mechanical suture; (4) both techniques were equivalent as to the tensile resistence, the deiscence, the fistula and the hemorrhage on the suture line, and (5) both methods were valid and safe for low rectum anastomosis
