Uso do Tramadol no controle da tosse crônica : estudo N-DE-1 e revisão sistemática

RESUMO

This work was developed from one Brazilian Hospitals case (Hospital Universitário de Brasília) in which the patient noticed a great improvement on her chronic cough (secondary to a interstitial pneumopathy) while using tramadol 50 mg 12/12h, an analgesic prescribed for pain. Hence, the patient has been using this medicine since then. Tramadol is a central-action opioid recommended to relieve moderate to severe pain that unexpectedly showed its positive effects in chronic cough control. However there are no previous reports in literature about its antitussive effects until now. This work used two methodologies: a N-of-1 randomized controlled trial to verify tramadol antitussive activity, and a systematic review of the tramadol and antitussive agents in order to investigate their efficacy in suppressing or relieving chronic cough. The N-of-1 trial showed that tramadol was effective in controlling the patients cough. This effectiveness has been evidenced by the significantly improvement (p <0.001) in the cough intensity (assessed by the patient in visual scale) and on her health state at all times when she was using the drug. The systematic review demonstrated that there are no clinical studies on the antitussive activity of tramadol. This work is the first systematic review aiming to evaluate the efficacy of antitussive agents. Seven drugs that reduced the cough frequency and intensity were identified: lidocaine, codeine, epinastine, terbutaline, levodropropizine, moguisteine and montelucast. It is still important to mention that the studies found faced some methodological problems, which we detach the diversity of the cough causes, lack of criteria for chronic cough definition, the variability of methods to quantify and analyze cough frequency and reduced follow up. Thus, further randomized and controlled clinical trials need to be conducted in order to demonstrate the efficacy of antitussive agents in the chronic cough control.
