Uso de mitomicina-C em cirurgia de estrabismo : influencia na produção de fibrose cicatricial em coelhos

RESUMO

Purpose: To evaluate the efficiency of mitomycin-C in limiting the postoperative inflammatory response and scarring after strabismus surgery in rabbits. Metbods: A prospective, two stage masked controlled trial was conducted. In the first stage, the fibrosis production at the extraocular muscle reattachment site was increased after inferior rectus recession in eight rabbits. In the second stage, mitomycin-C (O A mg!ml) was applied intraoperatively to the eyes of twenty-four rabbits with inferior rectus recession. As a control, contralateral eyes were treated with saline solution. In order to search for muscle insertion migration, immediately after surgery the distance from the muscle to the limbus was measured. Seven weeks later, the distance between muscle and limbus was measured again, exenteration was performed and the sites of muscle reattachment were processed for histological examinations. The sums of the areas of the granulomas seen in the extraocular muscle reattachment sites of control and treated eyes were compared, as well as the difference between immediate and late limbus-to-muscle distances. Results: There was no muscle insertion migration and no significant inhibitory effect of mitomycin-C on the fibrosis production oftreated eyes compared to that of control eyes was seen. Conclusions: The intraoperative use of mitomycin-C (OA mg!ml) was not effective in controlling the postoperative inflammatory response in rabbit eyes after extraocular muscle surgery. Our data do not support the hypothesis that MMC reduces postoperative adhesions after strabismus surgery.
