Uma contribuição ao debate sobre partos cesareos : estudo prospectivo no municipio de São Paulo

RESUMO

The longitudinal study here presented proceeded from a semi-intentional sample ofthree hundred and sixty women living in the Municipality of São Paulo, distributed according to the type of service (public or private) and natural parturition, who were interviewed in three moments of their puerperal pregnant cycle, being the first interview realized until the twenty second week of gestation, the second in the month before the probable date of childbirth and the last afier the delivery. The women considered eligible for participating in the research should comply with the following criteria: have a pregnancy diagnosis confinned; live in the Municipality of São Paulo; intend to give birth in a hospital/matemity hospital of this same municipality; have between 18 and 40 years of age; have gestational age up to the fifth month (inc1usively) of gestation; have had at the most two prenatal consultations (public service) and up to three consuItations in the private service. Nevertheless, as expected in a study ofthis nature, there was an effective loss of 17.2% in the interviews, the sample being reduced to only two hundred and ninety eight women at the end. The main goal of this work was to identify the variations in the desire on the preference of a type of childbirth, manifested by the pregnant women during two moments of their pregnant cycle,in contrast with the effective outcome of this process. Through the analysis structured according to the individual trajectories of these preferences, we attempted to understand the role perfonned by the social economic and demographic variables, the type of assistance sector to which this woman was attached and the number of previous childbirths, to identify characteristics and specifics that could suppose the existence of individual trajectories predominant in the preferences. Based on these issues and on the accounts given by the women, the intention of this work under the current debate in Brazil on the indices and indications for a Caesarean birth, is to present the distance occurred between the desire for one type of childbirth and its concretization.
