Sistemas locais de saude : uma via para o sistema unico de saude? o caso de São Paulo 1989-1992

RESUMO

This work is an attempt to understand the meaning of the LOCAL HEALTH SYSTEM (LHS) project, as a way of transformation of the national health systems in the third world countries. Since Brazil is our reference mark, we intend to understand the meaning of the LHS project as a strategy of implantation of the Unique System of Health (USH). It was made necessary the analyses 01 the confrontation stablished among several authors that consider the LHS proposal as a way to enable the Sanitary Reform, differently from others that bet on different alternatives from this same movement. We elected for this work the recent experience of implantation of the Sanitary Districts in São Paulo city, between 1989 e 1992, for belng a democratic government conjuncture, that incorporate actors that participated directly in the Sanitary Reform, and that intended to enable the USH. We worked with the idea that these aetors are individuals that formulate policies, and in this implantation of policies we used the coherence postulate by Mário TESTA, that determines a necessary relationship among the proposals, methods and transformation of the institutions that are responsible ,of th1s implantation. The method allowed us to recognize coherences and i ncohe rences i n the i mplantation of LHS i n São Paulo, and to conclude that although it was an advance in the construction 01 USH for having introduced changes in the tecno-assistencial model, and increased notoriousTy the substructure of services and municipal expenses with hea1th, it did not achieve to being a unique manager due to the 1imits of the process imposed by its own conjuncture
