Sistema de apoio à decisão em exames ortopédicos da coluna vertebral para auxílio nos diagnósticos fisioterapêuticos da região cervical e lombar

RESUMO

The DSS Decision Support System has been little diffused among Physical Therapy clinics throughout Brazil. All the tasks of filling in patients` medical records, evaluations, and follow-up forms are manually performed. In this context, a research involving Brazilian University Clinical was made to establish the DSS employed by them, in order to identify the need for development of a more effective tool to assist physiotherapists in the clinical decision support process, especially for the diagnostics for Physical Therapy treatments of the spinal column in orthopedic exams. The main aim of the research was the development of a decision support system, and a knowledge base validated by specialists which should be capable of simulating the patients` diagnostics. The research brought forward the most frequent pathologies which affect the spinal column, and focused the studies on the cervical and lumbar areas. It aimed at identifying the factors that can characterize the possible diagnostics, thus forming a knowledge base treated with use of the Artificial Intelligence Shell called Expert Sinta, and stored up in a Firebird database. With this system, it is possible for the user/student to simulate a patients symptoms and signs, and thus get information relevant to the confirmation of a possible Physical Therapy diagnostic. In order to gather the necessary knowledge base, a research was undertaken with the help and under the coordination of the Catholic University of the State of Paraná through its Physical Therapy Clinics therapists and professors. One hundred and seventy-seven medical records with Physical Therapy diagnostics of the spine, cervical and lumbar area were analyzed. These represented 26% of a global number of 663 medical records randomly picked out of the files dated 2003 2005. As Gold Standard were used the 34 production rules created and validated by Catholic University of the State of Paraná Medical-School Clinics Physical Therapists, representing the methods applied by the students in their learning process. By means of these rules, it was possible to simulate a diagnostic for each of the 177 medical records with an accuracy of 0.978 i.e. almost 1.0, which was an undoubtedly satisfactory result. This research sought the distribution (spreading) of the above system and of the knowledge base to the entire Brazilian academic environment, as well as to the physical therapy area, hoping that, in the future, many professionals and students shall contribute to the continuous growth and validation of this knowledge base.
