RESUMO

Postpartum depression (PPD) is the most common affective disorder in the puerperium. It affects between 13% of women, is associated to decrease in quality of life, disturbing in family dynamics, and can influence negatively in the mother-infant interaction. PPD is not recognized as a subtype of depression. However, both in research and in practice, a tradition exists to consider postpartum depression a distinct diagnosis. Some researches also assert that the symptoms of postpartum depression are qualitatively different from non-postpartum depression. A specific characteristic of clinical features is presence of obsessive-compulsive symptoms (OCS), and intrusive harm-related obsessional thoughts are especially common among women with PPD. In this study, a convenience sample of 400 new mothers was recruited from public and private medical institutions. They were interviewed using SCID-I for diagnosis PPD, YBOCS checklist for identify obsessions and/or compulsions, and MINI for diagnosis of obsessive-compulsive disorder (OCD). Twenty nine (7,3%) met diagnostic criteria for PPD. In our sample, previous or familiar history of psychiatric disorder, history of clinical disease, being separated, history of abortion and normal delivery was associated to the occurrence of PPD. Factors, such as obstetric complications in pregnancy or childbirth, inter-occurrences with the newborn, unwished pregnancy, and the socioeconomic condition did not show significant association with the PPD. Obsessivecompulsive symptom were reported by depressed mother (n=26; 89,6%) significantly more frequent than non-depressed mother (n=228; 61,5%), p<0,001. Aggressive, contamination and miscellaneous obsessions, and cleaning/washing and checking compulsion were the most frequent obsessive-compulsive symptoms in PPD. Of the 400 postpartum women, 36 (9%) met diagnostic criteria for OCD according MINI and 53,8% had comorbid OCD and postpartum depression. Our conclusion was that although obsessive-compulsive symptoms are common in postpartum mothers it is significantly more frequent in women with PPD. Ask for this symptons have to be a rotine by professional that take care of postartum women
