Seleção dos pacientes e doadores para transplante de medula ossea alogenico aparentado na unidade de transplante de medula ossea na Universidade Estadual de Campinas

RESUMO

Context: The feasibility of allogeneic bone marrow transplantation (ABMT) in a developing country was not demonstrated until now. Many adverse conditions including social and economic limitation may reduce the overall results in this complex and expensive procedure. Objective: To characterise the most important clinical, social and economic features of candidates for transplant and their potential donors and the influence of these factors in terms of Overall Survival. Design: Retrospective and exploratory analysis. Setting: Haematology and Haemotherapy Centre, Bone Marrow Transplantation Unit, State University of Campinas, Brazil. Participants: Refereed candidates to bone marrow transplantation in our Unit, from July 1993 to July 2001. All candidates had came from different Haematology and Oncology Centres from several States of Brazil. Results: Refereed 1138 patients, candidates to ABMT; median age 25 years (2mo-60yo); 684 (60,1%) male; clinical hypothesis were severe aplastic anaemia and haematological malignancies; 497/1138 (43,6%) candidates had full match donors; 352/1138 (30,8%) eligible candidates for ABMT. Only 235/352 (66.7%) were transplanted; 123/235 (52,3%) patients had 1 to 8 degrees of scholarship; the monthly familiar income ranged from US$ 60 (7%) to more than US$ 400 (36%); Overall Survival for CML, SAA and AML, 58%, 60% and 30% respectively. Conclusion: Overall Survival for most frequent haematological diseases is similar to those reported from International Registry, except for AML. This descriptive and exploratory analysis confirms the feasibility of ABMT in a developing country like Brazil
