RESUMO

Due to the increase of life expectation, itâs observed a greater incidence of cardiovascular events and a high prevalence of hypertension, which is the major changeable cardiovascular risk factor. The diagnosis of hypertension is based on several measurements of blood pressure (BP) in physicianâs office, and in elderly patients there is a special concern about these measurements. The home blood pressure monitoring (HBPM) is confirmed as a useful tool in the diagnosis of hypertension and in the hypertensive patientâs follow-up. The measures of BP taken by HBPM compared to those taken in physicianâs office, demonstrate that the pressure levels were interfered by the presence of the observer and the environment, and also, that the registers taken at home are significantly lower than those taken in physicianâs office. The casual BP is not able to identify those patients with masked hypertension (MHT) and white coat hypertension (WCH), however the HBPM, as well as the office blood pressure monitoring can be used to identify these situations that have distinct treatment and prognostic.The analysis of the data obtained during the studies in elderly patients demonstrates that the HBPM correlates better with lesions in target organ and has higher prognostic value to cardiovascular morbidity and mortality when itâs compared to the casual measurement. The usage of the HBPM in elderly patients is justified by the greater prevalence of WCH and MHT in this population, as well as by the importance of the diagnosis and control of the hypertension in this group
