Receptor soluvel da transferrina e parametros reticulocitarios na beta talassemia heterozigota e na anemia ferropriva

The soluble fonn of transferrin receptor (sTfR) has been pointed as an useful parameter to assess the iron status and erythropoiesis activity. Immature reticulocytes present high sTfR concentration. We tested the correlation between sTfR and reticulocyte parameters in iron deficiency anemia (IDA) and P thalassemia heterozygous (P thal hetero) patients. Laboratorial parameters related to iron status and reticulocytes were studied in order to establish the clinical use of them to distinguish both anemias. Reticulocyte measurements were obtained using a semi-automated analyzer and serum concentration of sTfR was determined by an immonoenzymatic technique. Fifty IDA and 41 Pthal hetero patients were studied. Reticulocyte count was significantly higher in p thal hetero group than in IDA and control groups. On the other hand, sTfR analysis showed increased values in IDA patients, when compared to pthal hetero group. The better parameter to distinguish both anemias was sTfR index, obtained by the ratio sTtR/ferritin leveI. Transport compartment was better evaluated by serum transferrin measurement than transferrin iron binding capacity (TIBC) detenninations. The association of serum iron with transferrin measurements (transferrin index) improved the accuracy of the transferrin test. The correlation between high immature reticulocytes and sTfR leveI was observed only in IDA group, suggesting that cellular iron deprivation is the main responsible factor for the sTfR synthesis stimulation in immature red blood cells. High sTfR values in p thal hetero patients reflect a degree of ineffective erythropoiesis in this hemoglobinopathy. We concluded that sTfR, ferritin and transferrin measurements are useful and precise parameters to discriminate IDA from beta thal hetero patients
