Reabilitação e reinserção no trabalho de bancários portadores de LER/DORT: análise psicodinâmica

RESUMO

This case study investigates the psychodynamics of the rehabilitation process of work capacity and work reinsertion of bank workers with RSI/WROD (Repetitive Strain Injury/Work-Related Osteomolecular Disease). It is grounded on the principles of the Psychodynamics of Work, which studies the worker s psychic health mediated by subjective experiences of pleasure-suffering at the workplace. Analyzed within this dynamics is the function of the strategies of individual and collective defenses used by workers to face hardships engendered by subjective work relations, change it into pleasure and maintain health in a normal status. In Brazil, bank work has undergone deep changes within the past years as a result of task automation and informatization. The consequences of such changes were reflected in the manner of working: pace acceleration, longer work journeys, demands for more qualification, an increase in the pressure for time and productivity as well as threats of job dismissal. Alongside with these came new ways of developing ill health as a result of the current configurations of the organization of bank work , as is the case with RSI/WROD. This pathology bears a somatic symptomatology. The process is triggered from the combination of the specific effects of the following factors: repetition, pressure for time and productivity. Such factors do not initially reach the articulations or tendons, but rather, adequate mental functioning. Once pain is the first symptom, permanence at work on the part of bank workers taken ill due to RSI/WROD is brought to light. The methodology used is grounded on the principles of the Psychodynamics of Work. Two male and two female bank workers participated in the study, including six professionals who are involved in the process of treatment and reinsertion of bank workers presenting with RSI/WROD. Clinical interview was used for information collection and the resulting data were analyzed according to the clinical analysis of work and analysis of the nucleus of sense. Rehabilitation of work capacity of bank workers presenting with RSI/WROD is regarded as a challenge due to the multiplicity of the factors present in its ethyology. Professional rehabilitation is predicted in the Constitution of 1988 and the conditions for entering the Professional Rehabilitation Program offered by the UTRP/INSS (respectively the Technical Union of Professional Rehabilitation and the National Health System) depend on the establishment of the causal nexus of the pathology with the work task and also the residual work capacity. The results of the analyses indicate that, from the point of view of bank workers, the rehabilitation of work capacity and job reinsertion have failed for a number of factors: the feeling of abandonment and exclusion from the workplace; for symbolic and material losses; for the weakening of the muscleskeleton system; for the chronicity of pain, which is related to the loss of defensive strategies, bank workers do not believe that they have been healed. For the professionals involved in this process, health rehabilitation and job reinsertion have failed for the following reasons: the bank workers do not want to be treated in order to obtain secondary gains from the disease; for the lack of discipline in the treatment, which leads to relapses; upon their return, the workers did not find psychosocial conditions for their permanence and health maintenance because of exclusion and disconfirmation of their functional limitations; the institution in charge of professional rehabilitation does not bear either a technical or financial structure to carry out a follow-up and supervision of reinsertion cases; the long-term displacement from the workplace makes returning difficult. Finally, the loss of defensive strategies, without which the bank workers are not able to mediate fear perception and, for this reason, the permanence of somatic symptoms undermines the attempt to return to work.
