Proposta de uma entrevista semi-estruturada para autopsia psicologica em casos de suicidio

RESUMO

From the literature, it is well known the retrospective evaluation seems to be a scientific way to collect data. In cases that require mode of death investigation, the retrospective evaluation is usually called psychological autopsy. Data collection has been done through interviews with people who knew the victim. The key items to a psychological autopsy for suicide cases are: precipitants and/or stressores, motivation, lethality, and intentionality. Although the authors point out the importance of the clinic experience for psychological autopsy, they do not mention definite strategies, but just some lists, suggesting areas or topics of investigation. An instrument to evaluate psychological autopsy should provide enough, and suitable information in order to allow a reasonable degree of agreement among evaluators. The Semi-Structured interview for Psychological Autopsy proposed in this paper consists of four evaluation modules to evaluate key topics associated to suicide. Criteria for choosing the subjects for the research have been identified through a pilot study. In order to evaluate the instrument applicability, a sample formed by 42 subjects related to 21 suicide cases has been used. Twenty-five (25) interviews have been tape recorded first, and then transcribed. The related material as well as the police inquiry data investigation have been presented to four judges. Evaluation of information has been carried out with the help of a four-module formulary for decision taken: precjpitants and/or stressores, motivation, lethality, and intentionality. Data processing, whenever possible, has been done through kappa statistics, from STATA software. Every time when kappa statistics was not possible to beutilized, agreement percentage has been used. The results of this study have shown that: a) it is possible to use a semi-structured instrument for psychological autopsy, in suicide cases; b) the Semi-Structured Interview for Psychological Autopsy (SSIP A) in suicide cases, theoretically based on four key topics, is applicable because it provides information containing a significant degree of agreement among evaluators, found in one hundred twenty measurements of judgements made by four judges. That is to say: a) there has been significant statistics agreement among the judges three modules of the model for decision, except in one step ftom each module, which did not interfere in the final agreement on the present evaluation; b) in the lethality evaluation module, there has been a 100% agreement among judges; c) the judgements made by four evaluators on the four modules have allowed to reach a final agreement related to suicide mode
