RESUMO

To register health information is a daily activity and is essential for proper health care delivery and patient safety, health personnel liability. The electronic medical records presents the possibility of adding, interconnecting and organizing such a range of information with different purposes in a intelligent way. The municipality of Fortaleza decided to computerize its primary health care network with an electronic health record (EHR) in 2002. The objective of this dissertation was to evaluate use and adoption by primary care physicians of the 6th heath district of the City of Fortaleza by examining satisfaction level, knowledge and usage of specific EHR functionalities; and by analyzing the physicians? perceptions on the implementation, ease of use, reliability, strengths and weaknesses of the EHR. It was an exploratory descriptive and evaluative research using a questionnaire as the instrument for data collection. All 111 active doctors in the primary health care network of the 6th heath district of the city of Fortaleza during the months of September and October of 2008 were invited to participate. The instrument included closed-ended and open-ended questions. We performed quantitative analysis on the use and satisfaction. Qualitative analysis was performed on the following themes: implementation decision, ease of use, system reliability and EHR strengths and weaknesses. The response rate was 89% (99 doctors). For overall satisfaction with the EHR, 2 (2%) were satisfied, 50 (50,5%) were satisfied in part and 47 (47,5%) were not satisfied. High use of the system functionalities was associated with the following variables: being woman (p<0.05), younger (p<0.05), seeing less than 16 patients per half-day (p<0.05), attending residency (p<0.05) and being less satisfied with the EHR (p<0.05). The results of qualitative analysis showed the following categories: EHR potential if it worked properly; high expectations for other EHRs; insufficient and inadequate infrastructure and support; ease of use, but low usability; inadequate for the purpose; dysfunction during patient care; lack of training and knowledge about the system; distrust in the data security; influence on physician-patient relationship. The conclusions were that primary care physicians in general were dissatisfied with current EHR and considered it not reliable. However, they believed that there are potential gains in using computerized systems. Therefore, the current EHR was shown to be inappropriate and should be reviewed or replaced. Paper medical records may be a temporary solution taking into account the context and planning of health services from the local primary care.
