O valor da punção aspirativa percutanea com agulha fina no diagnostico citopatologico de lesões pulmonares

RESUMO

Percutaneous Fine-Needle Aspiration: Value In the Cytopathologic Diagnosis of Lung Lesions. Percutaneous biopsy has been used in tile diagnosis of lung diseases for over a century but only recently it has gained wide usage mainly because of advances in cytopathologic methods, improved guidance modalities and utllizadon of fine-needles. In this country however, this procedure has not gained wide acceptance despite the obvious cost reduction. In the present study, using percutaneous fine-needle aspiration and based on 175 patients, the cyropathologic diagnosis was established in 130 (74,3%), the majority (64,6%) being malignant. A benign neoplasm was diagnosed in 2 (1,1 % ) patients and a nonmalignant lesion in 15 (8,5% ). The procedure was performed on an outpatient basis in 121 (69,2%) cases. In relation to cytopathological results of malígnancy, two false-negative results ocurred in patients with adenoma and, one false-positive was diagnosed (posteriorly confirmed as chronic abscess). Pneumothorax (22,3%) and hemoptysis (48%) were the only complications. The former required drainage in 10,9%. The spirometry data indicated that FEV1/FVC is a significant predicror of pneumothorax and the risk is greater when the test was listed as indicative of severe airway obstruction (% of predicted
