O uso previo de drogas anti-tiroidianas interfere na eficacia da radioiodoterapia na doença de graves?

RESUMO

Pretreatment with thionamides has been implicated in a reduced therapeutic effectiveness of radioiodine in thyrotoxicosis. In order to assess a difference between propylthiouracil(PTU) and methimazol e(MTZ) over patient s outcome, we examined 100 Graves disease patients 3, 6, 9 and 12 months after a 10 mCi standard single dose of 131I . They were assigned to one of 3 groups: no drugs treatment (n=30 cases), methimazol e (n=45 cases) and propylthiouracil(n=25 cases). Antithyroid drugs were with drawn 15 days before 131I .Goiter size was evaluated by pa1pation.We measured initia1TSH, free T4 (IT4) or T4, T3, TgAb, TPOAb, a nd 24 hours radioiodine uptake. Cure of hyperthyroidism was defined by evidence of euthyroidism or hypothyroidism in the absence of antithyroid drugs 12 months after treatment with 13 . The 3 groups were similar conceming age, sex, antithyroid drugs pretreatment duration, goiter size, T3, T4, IT4, TPOAb, TgAb, 24hradioiodine uptake and RAI dose administered per gram of thyroid tissue. Our resu1tsshow that no drugs and MTZ groups present a similar rate of cure of 73,3% and 77,8% (p= n.s.). Converse1yPTU group showed a rate of cure of just 32% (p<0,05). Our date agree with Imseis et ai. indicating that the use of PTU, but not MTZ, impairs the effectiveness of subsequent radioiodine therapy in Graves disease patients.
