O custo direto da fratura de fêmur decorrente de quedas em pessoas idosas: análise no sistema privado de saúde na cidade de Brasília, 2009

RESUMO

The main aim of this article is to estimate the direct cost of surgical treatment of femoral fractures resulting from falls during the hospitalization and after the surgery period up to two months after injury. The study was done in a transversal, descriptive and retrospective manner in patients of both sexes, aged 60 years or older, from January 2008 to June 2009. The data were extracted from hospital bills of a health insurance plan which uses the services of private hospitals in the city of Brasilia. The results showed that the incidence of femoral fractures in women was 76.2% (n=16) and 23.8% (n=5) in men, in which 66 7% (n=14) of the injuries resulted in proximal femur fracture. The average hospital stay per surgery was 7.1 days and 2.7 days in ICU. There were 16 patients with femoral fracture. The average expenditure per surgery was R$ 39.160,75, totalizing R$ 626,572.06. The direct cost of hospitalization was between R$ 8.293.55 and R$ 139,837.50, but the medical resources used in the post-surgery period was 5.7% of the direct cost of hospitalization. The study concluded that the cost per femoral fracture surgery is significant to the Brazilian private health system and that women are more vulnerable than men to this kind of injury. The biggest expenses were with prosthesis, material and medicine, and they vary according to the type of surgery, hospital stay period and age.
