Nurses perceptions of the care for hospitalized children with chronic diseases accompanied by a member of their families / A percepção do enfermeiro sobre o cuidado prestado a criança portadora de doença cronica hospitalizada em companhia de um familiar

RESUMO

This research investigates the work of the nurses who assist hospitalized children with chronic diseases, accompanied by a guardian, member of their families. This professional?s work with chronically ill children carries contradictions, conflicts and impasses that can be related to troubles in relationships established with the family or even to facing these children?s imminent death. Besides, these nurses need to have technical skills, be able to work with equipments and other scientifical innovations, as well as establish interpersonal relationship with her clients, which may be effectively therapeutic. Nurses, during their day-by-day work, also face institutional issues that directly influence their work. Thus, in this study, I aim at comprehend what is, for nurses, the meaning of taking care of hospitalized children with chronic diseases accompanied by a member of their families in a public school hospital, identifying which aspects are favorable and which are adverse in this nursing care. This is a qualitative research, exploratory modality. The field of research was a 48-bed rooming-in program Pediatric Ward. The participants were a group of nurses of this ward, whose number was defined by saturation. As soon as the research project was approved by the responsible Ethics Committee, data started to be collected. The technique used at this stage was the semi-direct interview, with an open-question guide. Two familiarization interviews were done, which were useful to better define the instrument of data collection that would be used; after that, six interviews were recorded, literally transcribed and then analyzed, according to Thematic Content Analysis. After some reading, I started the process of categorization. From this material, two broad categories may be apprehended ? ?The care to chronically ill children with focus on the disease? and ?The care to chronically ill children with focus on the children? ? which were analyzed according to the references of Health Work Process in the nursing assistance to children and adolescents. The nurses, while caring hospitalized chronically ill children focusing on the disease, demonstrate a sort of practice that is oriented to the realization of technical nursing and to the accomplishment of medical prescription. In relation to the guardian, concerning this process of care, he/she does what is determined by the nurses, it means, he/she is considered an agent in this work process. However, in this same situation, the participant nurses showed to take actions towards the chronically ill children, facing them as people. The interviewees start to recognize children?s important needs, as being stimulated, comforted and the need to establish relationships of trust with the ones who take care of them. In this kind of care that focuses the children, the guardian is recognized, at least initially, as someone meaningful to them, someone who suffers with their disease, who looks for adapting him/herself to a new way of life, who donates him/herself to the child, or draws away from him/her. However, in this study, this guardian is not considered as an object of nurses? care
