Melanoma oral e sinonasal primario : estudo clinicopatologico e imunoistoquimico / Primary oral and sinonasal melanoma : clinicopathological and immunohistochemical study

RESUMO

Melanoma is a potentially aggressive tumor of melanocytic origin. Only 1% develop in oral mucosa, accounting for 0,5% of all malignancies of the mouth. Despite the advances in biological knowledge of melanoma, treatment and prognosis still have limitations, especially in head and neck melanomas, including sinonasal and oral melanomas (OM). The aim of this study was to analyze the clinical, histopathological and expression of immunohistochemical markers in 33 cases of primary sinonasal and OM. Thirteen cases involved male patients and 20 cases (60.61%) were female, the mean age of the patients was 57,5 years, manifesting as nodular lesions, macular and bleeding areas, ulceration and numbness, especially in hard palate (45,45%) and upper gingiva (21,21%). Microscopically, four cases were amelanotic, with predominance of level III of invasion (23 cases) and polymorphous cell composition (20 cases) particularly composed of epithelioid and spindle cells. In some cases were observed necrosis, perivascular, perineural and adipose invasion and inflammatory infiltrate. Immunostaining results for S-100 and HMB-45 showed positivity in all cases, 27 cases (81,82%) and 30 cases (90,91%) with positive rate in more than 50% of tumor cells respectively. Three cases were negative for Melan-A, with 23 cases (69,70%) presented positive results in more than 50% of tumor cells. Ki-67 showed high expression with labeling index ranged from 15,51% to 63% positivity, with an average of 38,90%. CD68 showed seven cases (50%) with focal staining, two cases (14,29%) moderate and five cases (35,71%) intense.
