RESUMO

The implementation of the Family Health Program (FHP), in the 90s, raised the discussion of changing the health care model. The FHP, initially perceived as focused on the low income population, incorporates the principles and guidelines of the Health Surveillance and goes for the changing strategy of the health care model, which originated in the primary care in the construction of Public Health System (PHS). However, proposals for changing the health care model faces the challenge of changing health practices, which implies a redefinition of the object, means, activities, technical and social relations, organizations and their culture. One of the main contributions from PHS to FHP was pointing out that health is not only the absence of diseases, but it is rather related to life and work conditions, which means the redefinition of the object of health. However, this new object has not been routinely discussed by health professionals. This problem seems to be one of the main roadblocks in changing practice. Hence the questions of this paper: how to define this new object? What tools do health professionals have to deal with it? To answer these questions, a case study was conducted in two teams of the FHP in the outskirts of Salvador-Ba. This study aims at analyzing FHPs practices under theoretical framework of Health Surveillance. The theoretical indications are based on the work process theory in the health field. The main source of data in this research was interviews with health professionals in both FHP teams. The collected data indicated that the prime object of FHP is diagnosing diseases and health problems. FHP is not taking their practices accordingly to the object of health problem in the population and the guidelines and principles of the Health Surveillance and strategy of FHP, which consider the damage, the risks and determinant factors related to the health-disease approach. Also, collected data indicated that when professionals use some of recommended tools, they do so incipiently, using it only to learn about diseases and harms to public welfare.
