Fatores de risco para o desconhecimento e retardo para o conhecimento da sorologia para o HIV durante a gestaÃÃo em uma maternidade do Nordeste do brasil

RESUMO

BACKGROUND: The estimated overall HIV test coverage among pregnant women in Brazil is 62%, but only 41% in northeastern Brazil. This is an obstacle to prophylaxis against vertical HIV transmission. The aim of this study was to identify risk factors for unawareness of HIV serological status among pregnant women and determine the test coverage up to the fourteenth week of pregnancy. METHODS: Case-control study in a reference maternity hospital for high-risk pregnancy in northeastern Brazil. 485 puerperae were interviewed about knowledge of the results from HIV testing and biological, reproductive, sociodemographic, healthcare and health information variables. RESULTS: 21.65% said they were unaware of their HIV serological status during pregnancy (78.35% coverage). 87.42% brought their antenatal card, but the test results were only recorded on 48.58% of them. 22% said they knew the results by the fourteenth week. Multivariate results associated with unawareness comprised less than nine years of schooling (OR=2.92; P=0.006), living outside the stateâs capital metropolitan region (OR=4.11; P=0.001), test performed in the third trimester of pregnancy (OR=11.6; p=0.000); and lack of counseling about the importance of this test for prophylaxis against vertical HIV transmission (OR=2.31; p=0.022). CONCLUSION: HIV testing had a high coverage in comparison with previous studies in northeastern Brazil, but coverage prior to the fourteenth week of pregnancy coverage was very low. Less than nine years of schooling, living outside the stateâs capital metropolitan region, test in the third trimester and lack of counseling were associated with unawareness, irrespective of other factors
