Factors associated to the HIV infection in individuals assisted in Testing and Counseling Centers in Goiás and the Federal District: Multicenter Study / Fatores associados à soropositividade ao HIV em indivíduos atendidos em centros de testagem e aconselhamento de Goiás e Distrito Federal: estudo multicêntrico

RESUMO

Introduction: the supply of the anti-HIV test for the population in general is considered an important public health measure. In Brazil these tests can be performed confidentially in Testing and Counseling Centers (CTA) with no charge. The data collected in these health units compose the CTA Information Systems (SICTA). The HIV infection Epidemiological Surveillance from the data provided by this service network is an urgent necessity in Brazil because the characterization of the demand of asymptomatic patients permits to know recent patterns of risk situations involved in the epidemic. Objectives: to identify and to analyze the seroprevalence of risk factors associated wiht HIV infection in individuals seeking in TCA from Central Brazil, included the repeatead testers. Metodology: Multicenter cross-sectional, analytical study with individuals who were submitted to the anti-HIV test in ten CTA in Goiás and the Federal District. The study was performed on the records of the national base of the SI-CTA of the STD/Aids National Program. The data were analyzed through descriptive statistics and logistic regression was used to identify risk factors associated to the HIV infection in this population. The results were presented according to the demographic units of analysis: Midcountry, Goiania and Federal District and according to the retester conditions. Results: A total of 16,991 individuals who performed the test in the CTA of the region in the period from 2003 to 2004 were analyzed. The infection global prevalence was of 1.5% varying between the CTA and the gender ratio (H:M) was of 3.7:1. The risk factors differ according to the units of analysis. The infection was associated to the CTA of the Capitals, to age, to being a partner of an individual with HIV/Aids, a "sex worker/MSM/disguised", health professional or DU; to the retest; to having relations with both genders and having learned about the CTA through friends or users. In Goiania and Brasília the factors associated to the infection were: age, between 30 and 39 years old (Brasília) and between 40 and 49 years old (Goiânia); being a "sex worker/MSM/disguised" or a partner of a HIV/Aids infected person. In Brasília, bisexualism, being a retester; IDU; having learned about the CTA through friends or users and having STD in the previous year. Education was not associated to the infection, though in Brasília, having at least three years of education reduced the chance of a positive result. Conclusions: The results showed there are epidemiological differences between individuals according to the local of testing, evidencing a distinct process of geographic diffusion of the infection. The importance of the CTA as component of the HIV infection epidemiological surveillance system is evidenced as source of notification of new diagnoses and the need to implement the SI-CTA as tool for monitoring the infection in the country
