RESUMO

Introduction: Chronic urticaria is characterized by occurrence of weals daily or almost daily for a period of at least 6 weeks. Like other skin conditions, chronic urticaria can adversely affect quality of life. Because urticaria is a largely non-life-threatening, and any disfigurement is temporary, its impact may be underestimated by medical staff. Thus, it is important to quantify the patients perspective of the severity of their disease. Objectives: Assess quality of life in patients with chronic urticaria with one generic (SF-36) and one disease-specific (DLQI) health-related quality of life instrument. Compare quality of life and disability in different types of urticaria. Compare our results to other similar studies. Assess if associated angioedema impairs quality of life. Patients and methods: Sixty two patients were recruited from the Urticaria Clinics of Department of Dermatology of UNIFESP from may/2007 to february/2009. Using the DLQI and SF-36, patients were asked how their lives have been affected by chronic urticaria over the preceding week. Results: Of the 62 patients, who completed the DLQI and SF-36 over the study period, 72.6% were female and the mean age of subjects was 39.8 years. Duration of disease range from 2 to 256 months. We observed that 32.3% of patients have active disease for 1-5 years and 30.6% for longer than 5 years. Angioedema was associated with chronic urticaria in 75.8% of the patients. Subjects with ordinary urticaria associated with angioedema had greater quality of life impairment compared with subjects with physical urticaria and angioedema. Women also presented more angioedema associated to chronic urticaria than men. Among the studied patients 32.3% presented ordinary chronic urticaria, 27.4% physical chronic urticaria and 40.3% both types. The mean DLQI total score was 10.4. For DLQI, the subdomains Symptoms and feelings and Activities and clothing were more disabled. For SF-36, the more disable subdomains were Physical Role and Vitality. Women were more disabled in both questionnaires. Patients aged below 30 years were more disable only in DLQI. Subjects with chronic urticaria associated with angioedema had greater quality of life impairment compared with subjects without concurrent angioedema. Patients attending at Urticaria Clinics for the first time were more disabled as well as patients suffering from chronic urticaria until one year. Chronic urticaria patients were more disable in quality of life in both questionnaires than patients with rheumatoid arthritis, psoriasis, lupus erythematosus without active cutaneous lesions, comparing data in literature. Conclusions: The present study confirms a remarkable decrease of quality of life in urticaria patients. Chronic urticaria can cause significant quality of life impairment. Both instruments confirmed that chronic urticaria have an adverse impact on patients health-related quality of life. The differences between the mean scores in the different clinical types of urticaria were not significant. Angioedema did impaired quality of life. The estimated correlations between the instruments were in the expected direction and mostly significant.
