Estudo comparativo da hiperoxigenação hiperbarica em alças colicas isquemicas : trabalho experimental em ratos

RESUMO

The intestinal cicatrization is a complex issue. A bad cicatrlzation leads to high levels of mortality and morbidity. The understanding of the intestinal answers to the adversity situations is a highly researched issue. The hyperbaric oxygen-therapy presents a defined role in the treatment of a lot of affections. Some authors report, experimentally, intestinal anastomoses cicatrization improvement with its use under the presence of ischemia or noto However, the efIects of hyperbaric hyperoxia in intestinal ischemia in upright lug without anastomosis are still controversial. The effects of hyperbaric oxygen-therapy in upright colic lug were analyzed with three different levels of induced ischemia. It was proposed a mechanical study of the intestinallug by the pressure test of the rupture by liquid distension in order to check the quality of intestinal cicatrization. Eighty rats, distributed in four groups of 20 animals in each one, were used. In group 1 (G1), the control group, ischemia was not caused. The colic ischemia was induced in groups 2 (G2), 3 (G3) and 4 (G4). In G2, it was performed the ligature of the inferior mesenteric artery trunk and ofthe marginal arcade at four centimeters above the peritoneal reflection. In group 3, it was performed the ligature of the inferior mesenteric artery trunk, of the marginal arcade at four centimeters above the peritoneal reflection and of the marginal arcade at the leveI of the peritoneal reflection. In G4, the ischemia was broaden through the ligature of the following vessels: middle colic artery, inferior mesenteric artery and marginal arcade to the leveI of peritoneal reflection. Soon afier the operatory setting each group of twenty animals was divided into two subgroups of ten animaIs each: With Hyperbaric Chamber (CC) and Without Hyperbaric Chamber (SC). The animais of the four CC subgroups were placed in an experimental hyperbaric chamber in order to inhale oxygen at 100%, at two AbsoIute Atmospheres, for 120 minutes. Each session was repeated daily, at the same time, for a four-day consecutive period. In the intervals between the sessions the animaIs could breath environrnent air and allowed to water and ration. In the fifth day after the experiment, that is, in the fourth post-operative day, the animaIs were undergone to euthanasia. The animaIs of the four SC subgroups were kept in environrnent air during the tive days of the experiment. The hyperbaric chamber sessions were not applied. There was no death in any of the groups. The statistical tests applied were the Variance Analyze (ANOVA) and the Tukey Test. Considering the ischemia factor (p=0,00001),the four groups were difIerent among them Group 4 is more ischemic than group 3; group 3 doesn t differ ttom group 2;but group 2 is more ischemic, than group I. There was no statistical difference between the subgroups ee and se (p=0.3461).The result analysis of the work leads to conclude that: The hyperbaric oxygen-therapy does not present improvement on the induced ischemia in rats upright colic lug.
