Efeitos dos antidiabéticos orais sobre a resposta da glicemia durante o exercício físico aeróbio em indivíduos com diabetes mellitus tipo 2

RESUMO

The Physical Exercise (EF) has been considered one of them pillars of the treatment of the Diabetes Mellitus type 2, associated to the diet control and use of antidiabetic therapy. Despite the benefit of the acute reduction of the glicemia with the EF, he has been attributed to the some drugs, especially the sulfonylureas a bigger risk of hypoglycemia in the diabetic ones during and accomplishment of physical exercises. Justified for the effect of these drugs in increasing the basal concentrations of insulin and inhibiting the endogenous glucose production. For this reason the objective of the study was to compare the effect of the metformin versus sulfonylureas and metiformin combined on the reply of the sanguineous glucose during the aerobic physical exercise of moderate intensity in individuals with diabetes type 2 in the state postprandial, as well as, to evaluate the occurrence of hypoglycemia episodes. The sample was composed for twelve diabetic individuals of type 2, using antidiabetic medication (sulfonylurea and/or metformin) not treated with insulin. The individuals had been subdivided in two groups: seven in metformin use only (GM), and five with use of metformin and sulfonylurea (GSM). All had carried through ten sessions of aerobic physical exercise with intensity enter 50 to 70% of the maximum consumption of oxygen and had had the blood glucose measured in the beginning and to the end of 45 minutes. The reduction of the blood glucose levels after the EF in group GM was significant when considered the values of initial and final (p=0,0002), the same behavior was observed in group GSM (p=0,0000). When compared values absolute (mg/dl) between groups, group GSM presented reduction significantly big of blood glucose that group GM (p=0,001), however, when the reduction through the values of percentile was compared (%), it was observed that the reduction was not significantly different between the groups (p=0,07). Despite the study evidencing that the combination of the sulfonylureas with the metformin provided a bigger reduction in absolute values of the blood glucose when compared with the effect of the metformin, it is not possible to attribute to the sulfonylureas from this result a larger risk of hipoglicemia, therefore difference in the reductions of the blood glucose was not observed enters the groups in ratio to the initial values. Of this form, we conclude that in the carried through physical exercise in the state postprandial and it are of the peak of action of the medication, the different evaluated medications had promoted similar effect on the reduction of the blood glucose, and that the risk of the sulfonylureas in combination with the metformin of increase the hypoglycemic was equivalent to the one of the metformin.
