Detecção de hpv e análise de marcadores imuno-histoquímicos em adenocarcinoma de colo uterino: estudo da correlação com a resposta à radioterapia

RESUMO

Cervical câncer is the second most common cancer affecting female population worldwide and about 20% of such tumors are classified as adenocarcinomas. In order to improve therapeutic approaches for cervical adenocarcinomas, it is important to study potential prognostic and predictive markers for such tumors. Our study comprises HPV DNA detection, HPV-16 genotyping and immunohistochemical analysis of estrogen receptor, progesterone receptor, Ki-6 7 a n d c -erbB-2 overexpression in 43 adenocarcinoma samples. All the samples were obtained from patients treated with radiotherapy at Hospital Araújo Jorge, Goiânia-GO. Estrogen receptor was detected in 9 cases (21,0%), and progesterone receptor was detected in 12 cases (28,0%). Ki-67 nuclear expression was positive in 36 cases (83,7%), and c-erbB-2 overexpression was detected in only 1 case. HPV DNA was positive in 37 samples (92,5%), and HPV-16 DNA was demonstrated in 30 tumors (75,0%). Statistical analysis was performed in order to analyze the differences in the expression of diverse markers (RE, RP, c-erb-B2 e Ki-67), and HPV DNA detection, according to clinical parameters. No significant difference (p d0,05) was demonstrated, except for the Ki-67 expression. Ki-67 expression frequencies compared in patients who were alive at the end of the follow up (93,0%), and patients who died (61,0%) were significantly different. The majority of patients who were alive at the end of analysis demonstrated Ki-67 expression. By using log rank analysis, no significant difference was observed in overall survival of patients expressing the molecular markers analyzed in our study. HPV DNA detection didnt seem to influence overall survival either. The size of our sample, 43 patients, certainly influenced the putative effect of the parameters analyzed on patients overall survival.
