RESUMO

The present study analyzes the factors that interfere with the decentralizing of the actions of the Sanitary Vigilance in the municipalities of the State of Bahia under the System Full Management, identifying impasses and challenges in the assumption of their responsibilities as managers of the municipal health system. It is a kind of exploratory research, qualitative, descriptive in nature, using the case study as a research strategy. The data were collected by means of interviews and analysis of documents in the light of the decentralization concept as a political phenomenon, since it involves the transference of power form the State and consequently it induces conflicts in the relationships among the government areas of the Brazilian federation. The results of this research show that, although many obstacles for the structuring of municipal sanitary vigilances in the management of sanitary risk have been surmounted for the promotion and protection of health, there are still many challenges for the consolidation of the V.I.S.A in those municipalities. This is due to the chronic scarcity of resources besides the historic dispute of power between state and municipal organs regarding decision making processes about health actions and resources, while the power of municipalities to take on their management competencies in the health area is getting stronger. By aggregating empirical evidences about the health decentralizing process in sanitary vigilance, this study tries to contribute to the strengthening of this process in the State of Bahia, so that the strategies for the implementation of the V.I.S.A actions may be reorganized in Bahias municipalities and limits may be surmounted and formerly acquired advancements may be put forward.
